M5 A1 Personal Healthy Healer Plan
Healthy Healer Personal Plan :
1. Create one or two goals (only one is required) you would like to accomplish in the next 6 months related to your personal wellness.
2. What activities, ideas or handouts from modules 1- 5 inspired you? Review the resources you collected and decide what your goal (s) will be. For example your goals could be related to mindfulness, self talk, physical activity, nutrition, or spirituality. Regardless what goal you select, consider the following questions:
• What small steps can you take in the beginning?
• Who can help you, join you or support you?
• What will you need to be successful? What will keep you motivated?
• What specific activities or strategies will move you toward your goal?
• How will you know (or other people know) that you are successful?
3. Fill out the Healthy Healer Personal Plan (form) as much as possible. (Note: some columns might not be useful.)
